In hospitals, Step Down Units (SDUs) offers an intermediary level of care between the intensive care units and the general medical-surgical wards. The aim of SDU is to offer treatment to patients who are more severe than the usualward patients (Wilson, Kennedy, & Garratt, 2001).

I had a challenge managing risk factors for coronary heart disease in a patient with type 2 diabetes Mellitus. I could not offer the necessary treatment of hyperglycaemia to the patient, which could delay the start and advancement of microvascular problems. Enhancement of glycaemic control minimizes the possibilityof coronary heart disease (Wilson, Kennedy, & Garratt, 2001).

It was challenging for me to boost insulin sensitivity in the patient because I could not meet the required attention to diet and exercise. I learned that regularexercise and provision of a high fiber, low- saturated fat diet with consequent loss of weight develops insulin sensitivity.

After failing to suppress endogenous glucose production, I later discovered that Metformin minimizes glucose production, thus, reducing insulin levels. It also prevents the probable risk of sulphonylureas and atherogenic potential connected to exacerbation of hyperinsulinaemia (Wilson, Kennedy, & Garratt, 2001).

Acarbose, which inhibits the a-glycosidase enzyme,impedesdeprivation of complex carbohydrates, therefore, reducing the normal postprandial increase in serum glucose levels. When used with low-calorie, low-fat diet, acarbose enhances glycaemic control in a type 2 diabetes mellitus patient.

I learned about cardiovascular safety of Sulphonylureas, which speed up the death of hypoxic cardiomyocytes. Sulphonylureas lead to the production of insulin through blockade of the metabolism-regulated adenosine triphosphate-sensitive potassium channel in the pancreas.The use of Sulphonylureas is linked to increased clinical signs of ischaemia in the process of angioplasty and increased death in diabetes patients undergoing coronary angioplasty (Wilson, Kennedy, & Garratt, 2001).

It is important to note that a thorough glucose control is important in the management of risk factors for coronary heart disease in patients with type 2 diabetes mellitus, whose prevalence is high.
