N5526 Health Promotion Project Scholarly Paper Guidelines

Each student will be writing a Health Promotion Project (HPP) Scholarly Paper.  Using the Precede-Proceed model, students will develop, plan and evaluate a Health Promotion project.  The student will not and should not actually implement this project but will develop, plan and evaluate it using the Precede-Proceed model.  Each of the first two papers (Assignments 1 & 2) rolls into the final project.  As the semester progresses the student will receive feedback from the grading professor and will continue to improve the document.  The result (Assignment #3) will be a scholarly paper that encompasses all the student has learned and demonstrates the ability to incorporate the course concepts.  The final paper (Assignment #3) will include all 7 phases of the Precede-Proceed model as well as revised copies of Assignments 1 & 2.

FIRST STEP – Choose a health promotion topic of interest

You will need to go to Healthy People 2020 (Topics and Objectives) at https://www.healthypeople.gov/2020/topics-objectives to find excellent ideas for this project.  If you are a DNP student, consider using your chosen project for this paper.

As you consider your project, think about the questions below to help you decide on a topic.

  1. Who is your target audience? The audience needs to be a group of people or patients that you will be working with in the future as an advanced practice nurse or educator in a primary care setting not hospital/acute care. The audience should be community based, outside of the acute care setting. Your audience should not be other health care providers. Your project design will not be teaching the teacher (ie, other staff or faculty). Your topic should be related to your DNP or MSN track population (FNP, NE, WHNP, PNP, NNP, Adult, etc.).
  2. Where is the location or agency for the proposed project – this can be any agency close to where you live – a school, a Club, and Health Care Organization. Although you will not implement the project, you will need to actually speak with a key person at the location or agency who also can identify and support the need. You will need to be able to provide actual contact information for this individual for us to have on hand. Remember, you are developing, planning and evaluating this project so it is ready to implement sometime in the future. Again, you are not implementing this project.
  3. Is there a need for this health promotion topic in this location or agency? This needs assessment, due to time restraints, can be an informal survey or discussion with the administration or staff. If a formal needs assessment has been done then all the better!  Keep a listing of the questions that you used to complete your survey and the answers given.

 

  1. Could you complete this focused project during this semester? Make the project focused, as if it is to be designed, implemented, completed, evaluated and written about within a three-month period.

 

The student will plan the project and develop the presentation or intervention along with evaluating the project.  The intervention/education may take several sessions over several weeks, if indicated. One 30-minute seminar is not acceptable. Two-hour classes every week for 12 weeks is not acceptable as it is too unrealistic from a time perspective. This project is to be designed by “you” to deliver and present.  Previously designed courses or presentations are not acceptable. 

 

You cannot use a readymade program and utilize it even if the facility does not have the program.  However, if you find educational material such as handouts that are published regarding the topic then using them is permissible as long as you have the author’s permission.  This permission must be documented.

 

Do not select smoking cessation, hand washing, and skin sun safety as these topics have been exhaustively presented as projects and in the literature.

Students will need to complete the following assignments. The complete process has been broken down into 5 stages, each stage is due at a different time throughout the semester:

  • HPP Initial Proposal-Due 2/2/20
  • HPP Final Proposal-Due 2/16/20
  • HPP Scholarly Paper Assignment #1-Due 3/10/20
  • HPP Scholarly Paper Assignment #2-Due 4/7/20
  • HPP Scholarly Paper Assignment #3-Due 5/1/20

HPP Initial Proposal (10 points) – Due 2/2/20

Include the following in your HPP Scholarly Paper Proposal. Limit your submission to 400-600 words.  An abstract is NOT required in the proposals but IS required in Assignments 1, 2, 3.

  1. What is the “one” population that you are going to working with (Adult, Family, Pediatric….etc)? Remember, the focus will be on areas of primary care, not acute care settings.

  2. Where or how is your topic addressed in Healthy People 2020?
  3. What is a brief description of your project? Helpful tip: Your project should be working with a patient population, NOT other healthcare providers.
  4. What type of needs assessment/survey did you perform that would define the needs of the given population? (In this circumstance what individuals in position of authority such as a supervisor, public health official, community leader did you to speak to that support your idea for a community health promotion project). This is not hypothetical and needs to be completed.

  5. How are you going to evaluate the outcomes? (What type of system or questionnaire will you have in place to measure changes in knowledge or behaviors? Avoid the measurement of feeling.
  6. What kind of timeline are you going to implement that will allow you to plan, implement, stop, evaluate, close down the project and write about it within a three-month period? Be specific.
  7. Location of Project. Helpful tip: your project needs to be implemented in the community. (Acute care settings or hospitals are NOT appropriate)
  8. Name, address and contact information of director or contact person that might be willing to work with you in the future on your project idea.  

Simply use question & answer format for the proposals, starting each proposal with an introductory paragraph. The format for your proposals should be your title, introductory paragraph, each of the above questions restated and then followed by the appropriate answer.

 

APA format is required for in-text citations and your reference page.

HPP Final Proposal (10 points) – Due 2/16/20

Your professors will review your Initial Proposal and provide feedback. Comments & corrections will be made, and the student needs to make the recommended changes in this Final Proposal and submit with the suggested changes.

HPP Scholarly Paper Assignment #1 (20 points) – Due 3/10/20

Once you have received feedback from your grading faculty on your HPP final proposal, you are ready to begin writing your scholarly paper. This first stage will be addressing Phases 1 & 2 of the PRECEDE-PROCEED Framework by Dr. Green. These phases include the detailed discussion of the social assessment and epidemiologic, behavioral, and environmental assessments. As with all papers, APA format is required with appropriate in text scholarly citation and references. APA headings often aid students with organizing their paper.  This paper requires a title page, abstract, introduction, conclusion and reference page.  Keep in mind, your abstract and introductory paragraph should change with each paper assignment as the content will be different in each paper.

This assignment should range between 450 to 1000 words in length. Make sure that you are covering the topic requested (strangely enough this is where the most points are lost).   The Title page, Abstract and reference pages do not count toward the word count.

Phase 1: Social Assessment/Situational Analysis

This section has your needs assessment. The need for this project must be identified by someone or some organization. Please answer the following questions in your paper:

Did the organization express the need and how did you measure this? Was a survey completed? Who completed the survey? What specific aspect of the problem have they identified as needing fixed?  What is the biggest issue with this need, etc.?  There may be some references and appendices that you will attach (surveys, etc.).

Also reference HP 2020, which addressed the topic as a need in the leading health indicators.

Phase 2: Epidemiological diagnosis: “epidemiology is the study of health and disease in a society”

This is the classic epidemiological part. The cause, risk factors, causal factors, frequency, possible outcomes, treatments of the chosen disease need to be addressed. Include the basic disease process. Discuss how the disease can be prevented and include primary, secondary, and tertiary levels of treatment.  Also answer:

How significant is the disease and how does it warrant examination (rates, proportions, which includes morbidity & mortality, incidence, and what is a person’s relative risk to develop the disease). How is this a priority for this community?

Address behavioral and non-behavioral causes (or personal and environmental factors).  This should include the behaviors of the people whose health is in question, or the behavior of those who hold the resources whatever they may be.  The environmental factors can be modified such as support, behavior, health or quality of life and should include physical, social and economic factors. The non-behavioral factors include non-modifiable factors such as age, gender, existing disease, workplace, etc.

HPP Scholarly Paper Assignment #2 (20 points) – Due 4/7/20

The first step is to revise content from Assignment 1 as per faculty suggestions and submit the correct Phases 1 and 2 at the beginning of this next assignment. In a separate Appendix, list what corrections you made to previous content and where those corrections can be found. (An example would be:  Page X: Statement addressing the correction and/or change.).  This is a running assignment, and previous content must be submitted as an ongoing project development paper.

This second stage will be addressing Phases 3 &4 of the PRECEDE-PROCEED Framework by Dr. Green. This will be a detailed discussion of the educational and ecological assessment. You will need to address predisposing, reinforcing, and enabling factors as well as the development of the process objectives. You will also be writing about the administrative/policy assessment and intervention alignment.

Phase 3: Educational & ecological or organizational assessment

This phase consists of sorting and categorizing the causal factors previously identified into three categories:

  • Predisposing factors: these are the antecedents that provide the rationale for the motivation or behavior: knowledge, attitudes, beliefs, values, preferences, experience, etc.
  • Enabling Factors: these are the antecedents that enable motivation to be realized such as programs, services, and resources that are available, and how available they are
  • Reinforcing Factors: things that happen subsequent to the behavior and provide continuing reward or incentive for the behavior to continue; social support, peers, and significant other (SO) support

You should then think about the relative importance and changeability of the identified factors. 

Next you will identify and write the process objectives for the project.   In order to write good clear objectives, you may want to consult Bloom’s Taxonomy.  These objectives need to be specific, quantifiable and measurable.  Please see the McKenzie (pages 136-148) for details and examples. Read over these pages carefully and realize that this section is asking for process objectives not impact objectives. It is important to know the difference.

Students will also choose and apply a theory such as the Health Belief Model, Theory of Reasoned Action, etc. that is relevant to their project.

Phase 4: Intervention Alignment & Administrative/Policy Assessment

Phase 4 focuses on the administrative and organizational concerns.  Please answer the following questions:

  • What kind of support, financial and personnel, etc. does the program have?
  • What is the timetable for the project?
  • What polices either help or hinder your project?
  • Are there potential barriers to the project due to administration or organizational diagnoses?
  • What laws affect the project, limitations and constraints?
  • How do the objectives of the organization fit with your plan?
  • What is your budget? Even if you have everything donated, include what the project would cost if supplies and your time were not free.

HPP Scholarly Paper Assignment #3 (30 points) – Due 5/1/20

This final paper will contain 450 to1000 words of new material (phases 5-7 of PRECEDE-PROCEED) as well as the corrected versions of the first 2 assignments to create a complete and final paper about your health promotion project. The Title page, Abstract and reference pages do not count toward the word count.

This paper requires a title page, abstract, introduction, conclusion, and reference page.  Keep in mind your abstract, introduction and conclusion will change with each paper assignment as the content will be different in each paper.

Please include in an appendix stating the corrections you made from the 2 previous assignments and the page where the corrections can be found.  This should just be the same appendix as in the previous assignment but simply with added content to include corrections for Assignment 2.

The Final written assignment will include a final Title page, updated and final abstract, an updated and brief introduction, the 2 previous HPP assignment with recommended changes, followed by a new section addressing Phases 5-7, references, and appendices. All content should be in full APA format.

Step 5 & 6: Implementation & Process

This stage encompasses the project development and implementation.  In this phase describe the method and strategies chosen to implement your project & how you intend to meet the objectives (could include lecture, case studies, group discussions, demonstrations, videos or combination). Be specific, include your location, classroom details, anticipated participant details, and timeline. Include any supplemental materials (PowerPoints, Hand-outs) as an appendix at the end.

Phase 7: Impact evaluation

The impact objectives are the second level of objectives in the hierarchy of objectives and their relation to evaluation.  There are 3 different types of impact objectives, and you will need to address each one.  They include learning, behavioral and environmental objectives.  You are addressing the program’s effectiveness by addressing how it affected the learning, behavioral and environmental objectives (impact objectives). Please see the McKenzie text (pages 136-148) for details and examples.

Conclusion

Summarize how the project has evolved, stumbling blocks that were encountered and changes that need to be made. Do you have predictions on how to expand the project?
