In this Final Project, you have the opportunity to explore interprofessional practice and consider various aspects of relationship building within the field in order to support individuals with concurrent disorders. One goal of this paper is to increase your knowledge and understanding of the professional duties and limitations of each discipline and how that may impact service delivery.

As part of this Final Project, you will also develop a support plan that you review with the client for discussion and collaborative treatment planning. You are required to be innovative and creative to develop strategies for identified barriers.

Case study

You are working for a general social service agency that provides outpatient counselling for individuals seeking support for substance use issues. You receive a referral from a family physician for a 28-year-old woman (Juliet) who is currently experiencing an emotional crisis, as her live-in boyfriend has just left and she feels she cannot cope without him. She was living in another province. When her relationship ended she moved back home to live with her older sister, who is married and has two children. She had been working in the service industry as a sales clerk in various retail establishments. She did not finish high school and has no post-secondary training or education.

She tells you that she does not know what she wants to do and comes across as very quiet, shy with low affect. Her sister has told you that she has been “unhealthy” for quite a while. She does not eat healthy food, drinks a great deal of soda pop, and eats sugary snacks. She is overweight and has no motivation for adopting healthy eating or exercising habits. She sleeps a great deal and smokes both cigarettes and marijuana on a daily basis. She has been on medication for depression and anxiety in the past and her sister is not sure if she is taking this regularly. You discover that she has seen a psychiatrist in the past and has been to several counsellors but does not feel “connected” to them. At this point, she is very lethargic. Her sister has to prompt her to get out of bed and take care of her personal hygiene.

Part A: Gathering Information

You will begin by reviewing a case study above. Review this case and imagine that you are in the position of supporting this person in her care plan. Choose three disciplines to learn about and explore in the context of your role in assisting this young woman. How would you work in collaboration with these different perspectives? Explore in the context of this support.

Of the three disciplines that you chose. (Substance use counsellor, psychologist, nursing) consider the following activities and information that will require a review of relevant evidence/literature and consultation with services from your community in various disciplines to provide a rationale for your analysis. Start with Part A and once this is complete, you can move on to Part B in which you will draft a mock plan.

 

In addition, answer fully the following questions for Substance use counsellor, psychologist, nursing:

 

  1. What role does each discipline play in screening, assessment, and intervention for concurrent disorders?
  2. What is the practice philosophy around mental health or substance use for each discipline?
  3. What are the limitations for each of these professions in providing intervention and support?
  4. In your community, how would collaborative practice work—what is done well and why and what could be done better and why?
  5. If you were working with these disciplines, how would you support the development of a ‘”support plan”?
  6. What would each discipline consider in the “plan”?
  7. If you were to draft a collaborative plan, what might you include so that each discipline is “heard” in the process?
  8. What might you do as a practitioner in the field to overcome some of the historical and present barriers in terms of working collaboratively?
