I am among millions of Americans who are enrolled in the Medicaid program due to their circumstances, as well as their level of income. Medicaid can be classified as an integrated model because it is practiced in all states under the Federal Acts. The program has its own administrative rules, which are applied in all health care facilities. States can enhance Medicaid by setting standards, which would address circumstances under primary health care management. An integrated model ensures that Medicaid program is available in all primary health care facilities while referral and emergency treatments are undertaken effectively.
One challenge that I usually encounter while receiving my primary health care is lack of coordination. Medicaid is fragmented because primary care physicians do not share information about patients. Consequently, many consultants complain about inadequate information that they receive from referring physicians. Primary care physicians usually prefer to deal with private insurance holders rather than Medicaid beneficiaries. When a sick person is relocated from a nursing home to a hospital, he/she has a higher possibility of being treated for a wrong illness due to errors that may occur during the transition. Many health insurance plans in the US are fragmented due to low exploitation of electronic records an increased independence among health care providers.
